On one side is Team Demineralization, whose players try to pull minerals out of the tooth and create cavities. On the other is Team Remineralization, which works to rebuild the tooth and restore minerals that were lost. The analogy may sound simplistic, but it vividly describes what happens in our mouths day after day, hour after hour, at a microscopic level.<\/p>\n<p>The traditional model of cavities is called Keyes\u2019 triad: three factors must combine\u20141) the host (the teeth), 2) the microflora (bacteria), and 3) the diet (sugar). That framing tries to simplify something far more complex. In contemporary biological medicine we treat caries as multifactorial\u2014driven by many additional variables. The first shortcoming of the old model is its narrow focus on what happens only in the mouth. In a modern approach we must remember that oral health is tied to whole-body health\u2014and is influenced by it just as strongly.<\/p>\n<p>We begin a journey into the science of cavities that considers both oral and systemic health, aiming for a more effective model that addresses local factors (the mouth) and global ones (the body).<\/p>\n<p>That helps us meet all the players in the tug-of-war:<\/p>\n<ul>\n<li><strong>Chemistry<\/strong> \u2014 There is a constant exchange across the tooth surface, especially of minerals such as calcium and phosphate. These minerals continually leave and re-enter as building blocks of the tooth surface. Their departure favors cavities; their return favors rebuilding and renewal.<\/li>\n<li><strong>Acidity \u2014 pH<\/strong> \u2014 Measuring acidity helps characterize the tooth\u2019s environment and shows which side is winning\u2014that is, the direction of mineral movement.<\/li>\n<li><strong>Biology<\/strong> \u2014 The most influential players governing mineral turnover are bacteria. Contrary to popular myth, not all bacteria are harmful; some help protect oral health. The goal is balance between helpful and harmful species\u2014conducting the symphony in our mouths.<\/li>\n<li><strong>The \u201csecret sauce\u201d \u2014 saliva<\/strong> \u2014 Saliva is critical to remineralization and effectively sets the rules of the game. When we maximize what saliva can do, it can become our winning card.<\/li>\n<li><strong>Tooth structure (enamel)<\/strong> \u2014 The tooth is living tissue, with nerve canals, blood vessels, and fluid movement. That property can make the tooth nearly impermeable to bacteria\u2014or accelerate caries dramatically.<\/li>\n<li><strong>Nutrition<\/strong> \u2014 Diet is a double-edged sword: it can cause cavities or help prevent them.<\/li>\n<li><strong>Fluoride<\/strong> \u2014 Fluoride\u2019s effects on teeth are well known, but does it belong in a biological approach to cavity prevention? We will address that in this series.<\/li>\n<li><strong>Active ingredients in dental products<\/strong> \u2014 Ingredients in toothpastes and mouth rinses, for example, can tip the balance.
