(Listen via the image\/audio on the original post.)<\/em><\/p>\n<p><strong>Dr. Yarden Goldstein:<\/strong> Nice to meet you.<\/p>\n<p><strong>Zohar:<\/strong> It sounds like architecture\u2014level the mountain before building a house.<\/p>\n<p><strong>Dr. Yarden:<\/strong> That is where the problem starts. Most \u201choles\u201d reflect a broader imbalance\u2014diet, microbiome, and materials\u2014not only a local drill-and-fill story.<\/p>\n<p><strong>Zohar:<\/strong> So there is logic to that. When you say sugars\u2014do you mean croissants and breads too?<\/p>\n<p><strong>Dr. Yarden:<\/strong> Yes. Fermentable carbohydrates feed acid-producing bacteria. Frequency matters as much as amount. Refining the oral environment\u2014reducing constant sugar exposure, supporting saliva, and choosing kinder materials\u2014prevents the cycle of fillings that themselves need replacing.<\/p>\n<p><strong>Zohar:<\/strong> You said earlier the dentist must know the patient\u2019s oral cavity\u2014for example whether to fill a cavity or not?<\/p>\n<p><strong>Dr. Yarden:<\/strong> Exactly. Early demineralization can sometimes remineralize. Not every shadow needs a bur. We assess activity, hygiene, diet, and risk before intervening.<\/p>\n<p><strong>Zohar:<\/strong> It sounds like a face cream\u2014but for teeth.<\/p>\n<p><strong>Dr. Yarden:<\/strong> Yes\u2014remineralizing pastes and gels (for example CPP-ACP or related technologies) deliver calcium and phosphate to the surface. Abroad these are common; we use them when indicated alongside hygiene coaching.<\/p>\n<p><strong>Zohar:<\/strong> Are those pastes already available overseas?<\/p>\n<p><strong>Dr. Yarden:<\/strong> Yes, and some reach Israel through clinics or specialty channels. They are adjuncts, not magic\u2014they work best with diet and plaque control.<\/p>\n<p><strong>Zohar:<\/strong> Chewing gums\u2014are they widely used? Is xylitol one of those problematic sugar substitutes?<\/p>\n<p><strong>Dr. Yarden:<\/strong> Xylitol is different from many artificial sweeteners: oral bacteria cannot ferment it well, and it can reduce <em>S. mutans<\/em> counts with regular use. Look for high xylitol content; moderation matters for digestion. It is a tool, not a license for sugar grazing.<\/p>\n<p><strong>Zohar:<\/strong> So the only reason SLS is in toothpaste is foam?<\/p>\n<p><strong>Dr. Yarden:<\/strong> Largely yes\u2014the foam feels \u201cclean\u201d but does not clean better. SLS can irritate mucosa and trigger aphthae in sensitive people. Foam is marketing psychology; detergent-free or low-SLS pastes are kinder for many mouths.<\/p>\n<p><strong>Zohar:<\/strong> Like breaking up a demonstration with a water hose\u2026<\/p>\n<p><strong>Dr. Yarden:<\/strong> A vivid image\u2014harsh chemistry that disperses everything instead of supporting a balanced microbiome.<\/p>\n<p><strong>Zohar:<\/strong> Many people brush in the morning. Health shops now sell fluoride-free pastes.<\/p>\n<p><strong>Dr. Yarden:<\/strong> Brushing timing and technique matter more than brand slogans. Fluoride has a long caries-prevention record; some patients prefer alternatives for personal or medical reasons. We discuss risk, diet, and remineralization options case by case rather than dogma.<\/p>\n<p><strong>Zohar:<\/strong> When you talk vitamins\u2014diet or those tooth creams?<\/p>\n<p><strong>Dr. Yarden:<\/strong> Both layers. Fat-soluble vitamins (A, D, K2) and minerals support tooth and bone metabolism from inside; topical remineralizers work at the surface. Diet remains the foundation.<\/p>\n<p><strong>Zohar:<\/strong> Another problematic material\u2014amalgam fillings. What are they?<\/p>\n<p><strong>Dr. Yarden:<\/strong> Amalgam is a mercury-containing alloy used for decades. It is durable and cheap, but mercury raises biological concerns\u2014especially with vapor release during chewing, polishing, or removal. Biological dentistry prefers mercury-free materials and safe removal protocols when replacement is chosen.<\/p>\n<p><strong>Zohar:<\/strong> Over how many years is that mercury released?<\/p>\n<p><strong>Dr. Yarden:<\/strong> Release is ongoing at low levels and spikes with heat, abrasion, and bruxism. Individual burden varies with number of fillings, grinding, and detox capacity.
